It would allow further research into the specific effects of MDMA for PTSD, which would facilitate a better understanding of how MDMA can be useful in other conditions such as substance use disorders. Empathogens or entactogens are a class of psychoactive drugs that tend to produce experiences of emotional communion, oneness, relatedness, and emotional openness—ie, empathy or sympathy. MDMA is manufactured by safrole, a liquid extract from sassafras trees. The most popular way to convert safrole into MDMA is by synthesizing the 3,4-methylenedioxyphenyl-2-propanone (MDP2P) intermediate via the Wacker process or by isomerizing safrole into isosafrole by a strong base. Both processes oxidize either safrole or isosafrole into the MDP2P intermediate which then undergoes reductive animation to make a racemic mixture of MDMA. In the Merck patent, safrole was added to HBr to form a bromosafrole, which was then added to methylamine to produce racemic MDMA16.
A 2017 study found that high doses of R-MDMA administered in mice increased prosocial behavior and facilitated fear-extinction learning but did not produce hyperthermia or signs of neurotoxicity. This is thought to owe itself to the lower dopamine release R-MDMA displays relative to SR-MDMA. This result suggests that R-MDMA may be a safer and more viable therapeutic than racemic MDMA.55 However, more research is needed to validate this finding. Pharmacogenetics is a subdivision of pharmacology which focuses on the genetics behind the enzymes and other mechanisms behind the metabolism of substances. This area of focus is important for determining the effects of MDMA on users.
Side effects
More officially, this substance is called MDMA (short for 3,4-Methylenedioxymethamphetamine). Rave culture and electronic dance music culture are often characterized by a sense of harmony and acceptance. Users may take ecstasy and other drugs to enhance sensory perception and create feelings of euphoria.
It is best to consult a licensed health practitioner about any symptoms or conditions you have and to discuss the use of herbal supplements. Priority review is a status assigned to drugs that, if wiki mdma approved, would represent significant improvements in the safety or effectiveness of the treatment, diagnosis, or prevention of serious conditions when compared to standard applications. More than 20% of Veterans with PTSD also have substance use disorder, and about a third of veterans seeking treatment for substance use disorder also have PTSD. Veterans may find difficulties in readjusting to civilian life after military service, and with the additional component of PTSD, may develop substance use disorder through trying to cope. And since both types of medicines are used in treatment for PTSD, it is believed that MDMA – based on these shared features – may also be promising in the treatment for substance use disorder.
Medicinal Use & Potential Therapeutic Effects
While ‘ecstasy’ is the popular name for MDMA, the functional definition of ecstasy is any pill represented as MDMA on the street. Ecstasy pills are notoriously unreliable in content, more so than most other street drugs, and commonly contain either caffeine, ephedrine, amphetamines, other MDXX’s, DXM, or in rare cases DOX’s, and don’t necessarily contain MDMA or any psychoactive. They can also contain very neurotoxic substances (such as PMA, 4-CA, among many others). This problem has led to the development of simple MDMA Test Kits that may help give the user a general sense of the content of a pill. However, to be diagnosed with a substance use disorder, a person must meet specific diagnostic criteria for continued substance use despite negative consequences.
Contamination With Other Drugs
It’s also important to point out that MDMA-Assisted Therapy has not been found to increase the risk of substance use. Mithoefer’s study caught the attention of a group of military veterans, as PTSD is a common concern amongst veterans and is often highly treatment-resistant. Perhaps even more stunning, 83% of the participants no longer qualified for a clinical PTSD diagnosis (as measured by the Clinician-Administered PTSD Scale). A follow-up study with these participants three and half years later showed that about 75% had sustained their reduction of PTSD symptoms.
Al (2001) in which the metabolistic rate of CYP2D6 substrates is lower in Asians than in Caucasians because the genes that are responsible for the synthesis of CYP2D6 have a variation different from the typical wild type gene which codes for enzymes with slower functionalities 21. Again taking into consideration that MDMA may acts as an accumulator and an inhibitor, then if the CYP2D6 enzyme has a slower functionality as observed in Asians then users with this genotype have a higher risk of acute toxicity and fatality. However, for Caucasians who have the wild-type genotype that produces a CYP2D6 enzyme with normal-fast functionality, then their susceptibility to fatality and the intensity of pharmacological effects are lower than that of Asians. It is important to remember when looking at data that the genetic populations are still averages of the population and that every individual has different genetic makeup which affects the pharmacokinetics of the drug on an individual basis. MDMA is notoriously known for its recreational use at electronic dance music (EDM) events, more specifically scenes called “raves.” However many users like to take Ecstasy many other music events such as concerts, dance and party scenes where it may seem to enhance the perception of music. MDMA is usually taken in a pill form, although it can be crushed and snorted, and which the purity of the pill is usually unknown.

In most countries, the drug is a controlled substance and its possession and sale are illegal. Research suggests MDMA is potentially addictive, although more research is needed. Some people who use MDMA do report symptoms of an MDMA-related substance use disorder.10 These symptoms include continued use despite negative side effects, tolerance—which is needing to take increasingly larger amounts of a drug for the same effects—withdrawal side effects, and craving.
Although it is an illegal drug, many people all over the world have been using MDMA. MDMA is illegal, and there are reports that it can have negative health effects. MDMA was first proposed for scheduling by the Drug Enforcement Administration (DEA) in July 1984 and was classified as a Schedule I controlled substance in the U.S. on 31 May 1985; on an emergency basis. No double blind studies had yet been conducted as to the efficacy of MDMA for psychotherapy. Tolerance to many of the effects of MDMA develops with prolonged and repeated use.
Some scientists believe Ecstasy may be useful in therapy since it has been found to reduce fear in some individuals. Recently, there has been some research on if it can help people with severe PTSD confront painful memories during therapy appointments.171819 It may also have potential to help people diagnosed with cancer manage their anxiety about death. The study documented no drug-related serious adverse events, adverse neurocognitive effects or clinically significant blood pressure increases.
In an emergency? Need treatment?
Taking an adulterated drug can lead to unexpected and unwelcome side effects and may increase its potential health risks. Over the past decade, illegally made opioids like fentanyl have been increasingly found in the drug supply, and have contributed to a dramatic rise in drug overdose deaths in the United States. This effect can be lessened by relaxing, but can also be relieved by placing a hot flannel over the genitals to encourage blood flow.6869 However, MDMA causes water retention and dilution of electrolytes. Schedule I drugs have no official medical use and are highly addictive.
Two years later, he and David E. Nichols published the first report on the drug’s psychotropic effect in humans. They described “altered state of consciousness with emotional and sensual overtones” that can be compared “to marijuana, and to psilocybin devoid of the hallucinatory component”. Water intoxication symptoms typically show up when a person consumes more than about 3-4 liters of water within an hour.6263 Also, an important cause of death following MDMA use is hyponatremia, low blood sodium levels as a result of drinking too much water. Subjective effects include stimulation, anxiety suppression, disinhibition, enhanced empathy and sociability, relaxation, and euphoria. It is classified as an entactogen due to how it facilitates feelings of closeness with one’s self and others.
The pharmaceutical company Merck patented MDMA in 1914 as a compound that could have pharmaceutical value. It would be several decades before further drug development would take place. Additionally, some researchers have suggested that MDMA might be useful as a treatment for some mental conditions, such as PTSD and disordered eating. MDMA is more likely to lead to a serious emergency, like an overdose, if it’s been cut with another drug.
However, because MDMA affects brain chemicals, some researchers think it could have lasting effects on areas, such as cognition and mood. It’s important to note that recreational use is often much more frequent and higher in dose than therapeutic use. When used in a therapeutic setting, MDMA may enhance many aspects of talk therapy. Repeated studies show that MDMA can make it easier for someone to approach therapy with vulnerability and an open mind. It may be able to help those dealing with trauma to feel safe and capable of breaking out of harmful thought patterns.
Tests of confiscated molly show that it’s often mixed with other harmful substances, including methamphetamine or bath salts. MDMA is a drug that’s commonly thought of as a party drug and is known for producing feelings of pleasure, energy, and positive emotion, along with altered sensory perception. Other drugs in this chemical group include methamphetamine and cathinone. The Food and Drug Administration (FDA) has designated MDMA-assisted psychotherapy for people with PTSD as a breakthrough therapy. There are ongoing clinical studies to test how well this treatment might work. Current regulations have classed MDMA as a Schedule 1 controlled substance.
MDMA mainly affects neurotransmitters in the brain, specifically serotonin, dopamine, and norepinephrine. MDMA increases the release of serotonin, a neurotransmitter that regulates mood, sleep, pain, emotion, appetite, and other behaviors. MDMA is said to increase the activity of serotonin and also decrease the release of dopamine. The increase in serotonin is caused by the fact that MDMA inhibits serotonin to enter the reuptake site, causing and excess flow of serotonin. Some side effects include euphoria, decreased aggression, loss of self consciousness, enhanced senses, and enhanced empathy towards others. MDMA also has many physical side effectsSince MDMA increases the activity of serotonin, the brain becomes depleted of serotonin.
MDMA has been shown to cause sexual dysfunction, including erectile dysfunction and delayed orgasm (see subjective effects). MDMA (an abbreviation of 3,4-methylenedioxymethamphetamine), also called “Molly” or “Ecstasy,” is a lab-made (synthetic) drug that has effects similar to stimulants like methamphetamine. It is typically sold illicitly as colorful tablets with imprinted logos, capsules, powder, or liquid.
- In addition to this, MDMA contains a methyl substitution on RN, a feature it shares with methamphetamine.
- Ecstasy pills are notoriously unreliable in content, more so than most other street drugs, and commonly contain either caffeine, ephedrine, amphetamines, other MDXX’s, DXM, or in rare cases DOX’s, and don’t necessarily contain MDMA or any psychoactive.
- When addiction has developed, cravings and withdrawal effects may occur if one suddenly stops their usage.
- Likewise, adverse effects become increasingly likely with higher doses and may include addiction, severe injury, or death ☠.
- This effect can be lessened by relaxing, but can also be relieved by placing a hot flannel over the genitals to encourage blood flow.6869 However, MDMA causes water retention and dilution of electrolytes.
MDMA is often called molly or ecstasy and is frequently taken in pill or capsule form. MDA constitutes part of the core structure of the β-adrenergic receptor agonist protokylol. MDMA exhibits cross-tolerance with all dopaminergic and serotonergic stimulants, meaning that after the consumption of MDMA all stimulants will have a reduced effect. In December 2023, Lykos Therapeutics (formely MAPS PBC) submitted a New Drug Application (NDA) to the FDA for MDMA-assisted therapy for PTSD treatment. Because of MDMA’s documented efficacy in treating PTSD and additional potential to treat OUD, it could be an important new integrative approach to simultaneous healing of the two disorders.
